Our healthcare organization is seeking a skilled Nurse Practitioner to join our team.

Job Description:

We are looking for an experienced and compassionate individual to provide high-quality patient care as a Nurse Practitioner. This role will involve working closely with our multidisciplinary team to deliver exceptional patient outcomes.

The successful candidate will have the ability to assess patients, develop and implement individualized care plans, and provide education to patients and their families.

  • This includes assessing residents who are acutely unwell or require nursing interventions.
  • Supporting the care team to ensure care is safe and meets resident needs.
  • Liaising with families and various health practitioners.
  • Documenting care needs and interventions.
  • Optionally, dedicating time to assist with resident assessment and audits.
Key Responsibilities:

The key responsibilities of this role include:

  1. Providing comprehensive nursing care to residents, including administration of medications, wound care, and vital sign monitoring.
  2. Collaborating with other healthcare professionals to coordinate patient care.
  3. Developing and implementing individualized care plans that address the unique needs of each resident.
  4. Conducting regular assessments to monitor patient progress and adjust care plans as needed.
Requirements:

To be considered for this role, you must possess:

  • Registration with AHPRA as a Division 1 Registered Nurse.
  • Well-developed communication skills (written and verbal).
  • National police clearance.
