- Work collaboratively with customers, families, and other stakeholders to develop and implement personalized support plans.
- Assist with daily living tasks, such as personal care, meal preparation, and household management.
- Support individuals to access community resources, services, and activities that promote social inclusion and well-being.
- Contribute to a supportive and inclusive team environment, promoting a culture of respect, empathy, and understanding.
- Valid Driver's Licence and reliable vehicle.
- Satisfactory NDIS Worker Screening check or willingness to obtain.
- Exceptional communication and interpersonal skills, with a 'can do' attitude.
- Availability to work a range of shifts from Monday to Friday.
- Experience providing tailored support to individuals and their families (preferred).
- Experience providing support for Mental Health (preferred).
- Qualification in Aged Care, Disability, Mental Health or Community Services or working towards (preferred).
- Mobile phone for work use.
