About the Role

As a Community Health Nursing Specialist, you will play a crucial role in delivering high-quality care to clients in their homes and communities. Working collaboratively with our team of healthcare professionals, you will contribute to quality improvement initiatives and develop therapeutic interventions to support patient outcomes.

Key Responsibilities:

  • Provide nursing assessments and care plans that align with best practice standards
  • Work under the guidance of a Registered Nurse to plan and execute nursing tasks
  • Engage with clients and families to deliver person-centred care and education
  • Contribute to continuous quality improvement activities
  • Educate clients and families on health conditions and self-care strategies

Essential Criteria:

  • Enrolled as a nurse with the Nursing and Midwifery Board of Australia (or eligible for enrolment), with a current practising certificate
  • Strong communication skills and ability to work effectively in a multidisciplinary team
  • Organisational skills with ability to prioritise care and recognise clinical changes
  • Commitment to person-centred care and working with diverse populations
  • Experience in nursing care, demonstrating competence in established practice standards
