Dedicated Home Support Worker

We are seeking a dedicated and compassionate Personal Care Assistant to join our team. As a key member of our care team, you will provide high-quality personal care and support to clients in their own homes.
">Key Responsibilities:
">- Provide personal care and assist with daily activities, following the client's care plan.
- Work closely with Registered Nurses to deliver client-specific care, including personal hygiene, toileting, nutrition, mobility, and more.
- Identify client care needs and work collaboratively with the client's care team to ensure high-quality, holistic care.
- Communicate effectively with clients, their families, and team members.
- Support clients with light housekeeping duties to promote independence, including laundry, cleaning, and meal preparation.
- Maintain accurate daily client notes and promptly report any concerns to the RN/Care Manager.
Qualifications and Essential Requirements:
">- Cert III in Aged Care/Disability (or equivalent).
- Current Senior First Aid [HLTAID011], CPR [HLTAID009] and Manual Handling [HLTWHS005] (with 3+ months until expiry).
- A roadworthy and insured vehicle with a current driver's licence.
- NDIS Screening (or willingness to get).
- Strong expertise in mental health care.
- Willingness to travel and flexibility with shifts.
- Strong communication and interpersonal skills with empathy for elderly clients.
- Knowledge of relevant legislation and standards for home care.
- Commitment to safety, infection control, and emergency procedures.
