Nursing Professional

This role is focused on the development of comprehensive nursing strategies that aim to improve health outcomes through population-based initiatives.

Key Responsibilities:
  • Develop and implement advanced nursing processes to identify high-risk patients and those with chronic conditions.
  • Collaborate with multidisciplinary teams to coordinate services and resources for patient populations and their families.
  • Assess, plan, educate, monitor, evaluate, and coordinate services to foster quality, continuity, and appropriate utilization of healthcare resources.
  • Facilitate the coordinated utilization of resources for maximization of health outcomes, patient/family satisfaction, and financial outcomes.
Required Skills and Qualifications:
  • Bachelor of Science in Nursing (BSN) or equivalent degree.
  • At least three years of clinical experience in care coordination, with strong knowledge of diabetic populations and other chronic diseases.
  • Experience with electronic medical records software and population health data analysis.
  • Strong communication and interpersonal skills, with ability to work effectively in a team environment.
  • Knowledge of quality improvement/performance improvement tools and proficiency with computer software programs.
