Job Title:

The Aftercare Lead is a vital role in supporting the delivery and coordination of aftercare services across the district.


Key Responsibilities:
  • Collaborating with stakeholders to embed and strengthen aftercare pathways
  • Coordinating stakeholder engagement and service integration
  • Supporting referral pathways and service improvements
  • Contributing to data reporting, evaluation, and quality initiatives

Requirements:
  • Tertiary qualifications and/or relevant experience in mental health and suicide prevention
  • Experience working collaboratively with diverse stakeholders
  • Ability to contribute to embedding best practice approaches and improving referral pathways
  • Capacity to support service design and implementation, and contribute to service evaluation or data reporting
