Job Description

The role of a Rehabilitation Nurse is centered around providing specialized care to patients undergoing rehabilitation for various medical conditions, injuries, or surgeries.

Key Responsibilities:

  • Patient Assessment: Conduct comprehensive assessments of patients' physical, cognitive, and emotional abilities to determine their rehabilitation needs and goals.
  • Care Planning and Implementation: Collaborate with interdisciplinary teams to develop individualized care plans tailored to each patient's specific needs and rehabilitation goals.
  • Rehabilitation Therapy Support: Assist patients with rehabilitation exercises, activities of daily living (ADLs), mobility training, and adaptive equipment use.
  • Patient and Family Education: Provide education to patients and their families about their medical conditions, rehabilitation goals, and self-care strategies.
  • Psycho-Social Support: Offer emotional support and counseling to patients and their families throughout the rehabilitation process.
  • Assistive Device Management: Assess patients' needs for assistive devices such as wheelchairs, walkers, orthoses, and prostheses.
  • Wound Care and Skin Management: Monitor patients for skin breakdown, pressure ulcers, and wound healing during the rehabilitation process.
  • Safety and Functional Mobility: Promote safety and functional mobility by implementing fall prevention strategies, assisting with transfers and mobility exercises, and assessing patients' home environments for safety hazards.
  • Discharge Planning and Transition of Care: Collaborate with patients, families, and healthcare providers to develop discharge plans and coordinate post-rehabilitation care and services.
