The Service Navigator exists to improve accessibility to the health system for individuals with severe mental illness and associated psychosocial disorders. We engage with and advocate for individuals, connect them with appropriate support networks, and empower them on their recovery journey.
Working closely with Local Health Networks and NDIS Local Area Coordinators, we facilitate consumer access and enhance coordinated referral processes through creation and distribution of detailed resources relating to community-based support services as well as psychosocial, clinical, and primary healthcare.
To be successful, you will work alongside our Psychosocial support programs and intake teams. As a Service Navigator, you will build and draw upon our Community Recovery Model to establish and enhance coordinated referral processes and community support services focused on psychosocial support needs such as housing, employment, education, family support, and financial matters.
Key requirements include relevant qualification minimum a Diploma in Counselling or relevant Mental Health qualification or above, commensurate experience in working in a support role for people with mental illness, current valid Driver's License, evidence of the right to work in Australia, National Police Records Check, Working with Children Check, Employment, NDIS Worker Orientation Module certificate, and NDIS Worker Screening Check clearance.
