First Response Case Worker
About This Role:This role offers an exciting opportunity to make a meaningful impact in the lives of individuals and families at risk or experiencing homelessness. As a First Response Case Worker, you will be part of a close-knit team providing person-centred support to those in need.
The Salvation Army Australia's Homelessness Stream provides high-quality services to people who are homeless or at risk. We support adults and families, including accompanying children, with accommodation, case management, outreach support, financial assistance, connection, and referral to other specialist services.
You will bring your innovation, adaptability, resilience, networking, and time management skills forward to provide effective support to our community members. Our team works collaboratively to provide a comprehensive response to participant needs, and this position will be predominantly based in Moreton Bay but will work in multiple locations across the region.
Key Responsibilities:- Initial Assessment and Intervention: Provide timely and accurate responses to calls, answer questions in plain English, and complete comprehensive initial assessments and intake that promote participation, self-determination, and connection to the community.
- Case Management: Develop case plans alongside participants, which reflect their needs, goals, and interventions to address barriers. Continually assess presenting issues, vulnerabilities, and needs from initial intake to exit.
To be successful in this role, you will have:
- Tertiary qualification in Community Services and Social Work (desirable).
- Relevant experience in a social service environment, including client assessment and case management.
- Prior work experience or training in Domestic & Family Violence (highly regarded).
- Prior work experience or training in Mental Health (highly regarded).
- Previous experience providing case management type support over the phone (preferred).
