About the Role:

As a care facilitator, you will visit clients in their homes, assisting them and their families to engage with services that enable them to remain living at home.

  • You will develop strong relationships with clients and their families, acknowledging their needs and working closely with them to set realistic goals.
  • You will implement best practice approaches for care, working as part of a multidisciplinary team to deliver high-quality support.
Key Responsibilities:
  1. Home visiting clients who have been referred for help at home
  2. Developing relationships with families and carers that acknowledge their needs
  3. Working alongside clients to set individual goals that are achievable
  4. Implementation of best practice and evidence-based approaches for care
What We Need From You:
  • You must be comfortable working on your own in the community
  • You need excellent verbal communication skills
  • An empathetic, patient and caring nature is essential
