About The Role

Our Emergency Team Care Coordinators provide expert assessment and care planning for older adults to prevent hospital readmissions. They work closely with healthcare professionals to develop clinical pathways, protocols, and models of care that are informed by evidence-based practice and research.

This is a multidisciplinary role that is open to registered nurses and allied health professionals. If you have a passion for providing high-quality patient care and contributing to the development of best practice in emergency settings, we encourage you to apply.

Key Responsibilities
  • Provide expert screening, assessment, and care planning for older adults
  • Work collaboratively with healthcare teams to develop and implement clinical pathways, protocols, and models of care
  • Contribute to quality improvement and risk management initiatives within the area of practice
Required Skills And Qualifications
  • Current registration with AHPRA or other relevant peak professional body
  • Specific postgraduate qualification and 12 months' experience working in the clinical area related to the postgraduate qualification
  • At least five years' relevant clinical experience
  • High-level clinical leadership skills
  • Ability to contribute to quality improvement, risk management, and research projects within the area of practice
