Job Description:

The successful applicant will be responsible for developing and implementing individual housing plans and exit strategies for men residing at the Maribyrnong Community Residential Facility (MCRF), as well as providing support in establishing long-term housing options in the community.

Responsibilities:

">
  • Work closely with the case management team and Senior Practitioner to support day-to-day operations of the Transitional Support Program
  • Manage a caseload of participants with complex needs and histories of incarceration
  • Ability to work independently in the community in an outreach capacity
  • Support participants to develop tailored housing plans and establish stable housing in the community
