Expert Advanced Practice Clinician

Job Description:
- To deliver high-quality clinical nursing care with autonomy and collaboration to provide patient-centered care within the Supportive and Palliative Care service.
- To champion, educate, and lead a multidisciplinary team to enhance patient outcomes through best practice problem-solving and decision-making.
- Ensure seamless care coordination across the continuum from hospital to home.
- Communicate effectively with patients, families/carers, and healthcare providers.
- Current registration with AHPRA – Division 1 Registered Nurse.
- Post-registration qualifications in palliative care.
- At least 3 years full-time equivalent experience in palliative care nursing.
The successful candidate will demonstrate commitment to championing, educating, and leading a multidisciplinary team, effective communication skills, and dedication to clinical practice development.
