Clinical Nurse Specialist

Job Description:

  • To deliver high-quality clinical nursing care with autonomy and collaboration to provide patient-centered care within the Supportive and Palliative Care service.
  • To champion, educate, and lead a multidisciplinary team to enhance patient outcomes through best practice problem-solving and decision-making.
Responsibilities:
  • Ensure seamless care coordination across the continuum from hospital to home.
  • Communicate effectively with patients, families/carers, and healthcare providers.
Qualifications:
  • Current registration with AHPRA – Division 1 Registered Nurse.
  • Post-registration qualifications in palliative care.
  • At least 3 years full-time equivalent experience in palliative care nursing.

The successful candidate will demonstrate commitment to championing, educating, and leading a multidisciplinary team, effective communication skills, and dedication to clinical practice development.
