Role Summary

We are seeking a skilled Social Worker to join our team in Regional Western Australia. This is an exciting opportunity to work in a generalist hospital role and contribute to the planning, development, implementation, and evaluation of the Social Work service.

The successful candidate will have strong communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare professionals.

About the Role

As a Social Worker, you will be responsible for:

  • Delivering patient-centred care, building health literacy, and promoting self-management.
  • Coordinating the planning, development, implementation, and evaluation of the Social Work service for inpatients, outpatients, community, and Child Development Services.
  • Providing advanced level clinical care for social work clients and identifying priority health issues and high-risk groups requiring specific programs.
  • Participating in inter-professional and interagency collaboration for clients, as appropriate.

Requirements

To be successful in this role, you will need:

  • Australian Association of Social Work (AASW) registration.
  • Hospital experience.
  • A minimum of 2 years post-qualification experience.
  • A Working With Children Check (WWCC).
  • A National Police Check.
