Full Time Social Worker

We are seeking a skilled Social Worker to join our team in the Adult Combined Locality Team. The successful candidate will be responsible for providing psychosocial assessments and complex discharge planning for patients in oncology wards.
The ideal candidate will have a degree in Social Work, current or eligible AASW registration, and strong skills in discharge planning and assessments.
Additionally, the candidate should be fully vaccinated for COVID-19 (including booster) and possess excellent communication skills.
As a member of our team, you will have the opportunity to work closely with medical teams to deliver patient-centred care in a supportive environment.
