We are seeking a dedicated and skilled professional to join our team as a Community Support Specialist.
This role involves supporting individuals with disabilities through the National Disability Insurance Scheme (NDIS) journey, focusing on scheme access, plan development, and implementation of funded supports.
The successful candidate will have experience in community services, social work, or a related field, with a strong understanding of the NDIS participant pathway and planning processes.
Key responsibilities include managing caseloads, meeting service delivery targets, and ensuring timely and high-quality service delivery in line with performance targets.
The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work independently in community-based settings.
