Care Coordinator Opportunity

The role of the care coordinator involves delivering timely and comprehensive assessment and care coordination of patients with complex needs to facilitate patient-centred care, effective management of the ED episode of care and support discharge planning and community integration.

  • This position will be embedded into a mental health and addiction hub team and report directly to team leaders.
  • This position will also have strong links to an existing emergency department care coordination team.

Care Coordination Responsibilities:

  • Respond and screen eligible clients for the mental health and addiction hub
  • Provide clinical response to eligible mental health and addiction hub patients
  • Provide appropriate discipline-specific clinical management, discharge planning & community liaison for all relevant patients in collaboration with team members within the hub
  • Facilitate appropriate admissions/nominations to inpatient and outreach services
  • Ensure consumers receive information in an accessible format
  • Actively support consumers to make informed decisions about their treatment and ongoing care
  • Ensure consumers are aware of their rights and responsibilities and how to provide feedback

Working Together

The multidisciplinary team brings together clinicians from mental health, drug and alcohol and the emergency department care coordination team to provide specialised care to patients presenting to the emergency department with high acuity mental health and addiction issues and complex psychosocial needs.
