
Making a Difference: Aboriginal Health Pathways Worker

This is a dynamic opportunity to make a meaningful difference in the lives of children and young people in out-of-home care.
We are seeking an experienced Aboriginal Health Pathways Worker to join our multidisciplinary health service, which provides tailored health management plans for this vulnerable population.
The successful candidate will assist children, young people, and their families in navigating healthcare services and overcoming barriers to access. This role involves building strong relationships with local First Nations healthcare providers and other stakeholders.
Key responsibilities include:
- Identifying and addressing obstacles to healthcare access for children and young people.
- Coordinating with external services such as dental, maternal and child health, and child protection services.
- Providing expert input on multidisciplinary assessments for First Nations children and young people.
- Making referrals based on assessment outcomes and supporting access to these services.
Successful applicants will have knowledge of and experience working with the local Aboriginal community and culture. They will also possess excellent communication skills, be able to work effectively and sensitively, and maintain confidentiality and privacy.
