Clinical Care Coordinator Position


We seek a highly experienced Clinical Care Coordinator to lead our residential aged care operations, ensuring residents receive exceptional care aligned with quality standards.

Key Responsibilities:

  • Care Planning: Organising case conferences to partner with consumers and their representatives in developing tailored care plans;
  • Expert Case Management: Collaborating with the Residential Care Manager to deliver high-quality patient care;
  • Quality Improvement: Monitoring and enhancing clinical outcomes through regular audits and initiatives;
  • Patient Advocacy: Supporting policy development and implementation for resident care;
  • Staff Development: Leading, coaching, and inspiring staff to deliver outstanding care.

About You:

  • You must be a Registered Nurse with current AHPRA registration;
  • A proven track record of experience in residential aged care;
  • Clinical Skills: Strong skills in assessments, care planning, and change management;
  • Funding Knowledge: Sound understanding of aged care funding models;
