Clinical Care Coordinator Position
We seek a highly experienced Clinical Care Coordinator to lead our residential aged care operations, ensuring residents receive exceptional care aligned with quality standards.
This role is ideal for individuals with strong leadership and change management skills, who are passionate about enhancing people, culture, and care outcomes.
Key Responsibilities:
- Care Planning: Organising case conferences to partner with consumers and their representatives in developing tailored care plans;
- Expert Case Management: Collaborating with the Residential Care Manager to deliver high-quality patient care;
- Quality Improvement: Monitoring and enhancing clinical outcomes through regular audits and initiatives;
- Patient Advocacy: Supporting policy development and implementation for resident care;
- Staff Development: Leading, coaching, and inspiring staff to deliver outstanding care.
About You:
- You must be a Registered Nurse with current AHPRA registration;
- A proven track record of experience in residential aged care;
- Clinical Skills: Strong skills in assessments, care planning, and change management;
- Funding Knowledge: Sound understanding of aged care funding models;
- Commitment to Excellence: Commitment to best practice and continuous improvement.
