Clinical Nurse Consultant for Aged Care
Job Description:
The clinical nurse consultant plays a pivotal role in coordinating hospital avoidance strategies for people aged 70 and over residing in residential aged care facilities and the community. This entails providing leadership and expertise to develop comprehensive plans aimed at preventing hospital presentations or timely returns to residents' residences with supportive strategies in place.
Key Responsibilities:
- Coordinate fast-track diagnostic, assessment, and treatment processes to avoid hospital visits or expedite safe returns to residents' homes.
- Provide effective short-term case management for older individuals while enhancing capacity within RACF staff through evidence-based practice promotion and advanced care planning.
Expertise and Leadership:
The clinical nurse consultant is an expert professional who offers guidance, recommendations, and leadership on all aspects of nursing. They consult on matters related to the safe and appropriate use of nursing practices, formulating and implementing policy, guiding practice development, and engaging in education and research activities.
Supervision and Mentorship:
Responsible for providing clinical supervision to nursing staff, serving as a role model by delivering regular nursing care, and fostering a culture of excellence in patient care.
