Visits to patients' homes are included in this position, requiring a reliable vehicle.
- Key Responsibilities:
- Conduct comprehensive assessments of patients and develop treatment plans.
- Prescribe medications and treatments in accordance with employer policies and procedures.
- Collaborate with other healthcare professionals to identify and assess trends and patterns that impact client outcomes.
- Develop and implement population-based strategies to improve health outcomes.
- Participate in peer review and evaluate the effectiveness of services at the client, community, and population levels.
- Engage in ongoing professional development and maintain competence in the role.
- Report to an Operational Director and contribute to quality assurance and patient safety initiatives.
