**About the Role:**

We are seeking a highly skilled and motivated Community Care Coordinator to join our team. The successful candidate will be responsible for coordinating, care planning and monitoring of CHSP clients to ensure a quality service is provided.

Key Responsibilities:

  • Coordination of CHSP services for clients
  • Care planning and monitoring of clients
  • Building relationships with clients, their families and other health professionals

Requirements:

  • Minimum 1 year experience as a CHSP or HCP Coordinator
  • Diploma of Community Services or equivalent qualification
  • Current First Aid and CPR certification
  • Strong organisational skills including problem solving, prioritising and multi-tasking
  • Excellent communication and interpersonal skills
  • Good computer skills including care databases, word, excel, outlook
