Community Care Coordinator

We are seeking an experienced coordinator to join our Community Care team in Queensland, Australia.

Key Responsibilities:

  • Assess and coordinate care delivery for clients, ensuring they maintain independence and develop skills.
  • Enable clients to achieve optimal health outcomes by providing direct support and guidance.
  • Monitor and report changes in client behavior or reactions, documenting progress and adjusting care plans as necessary.
  • Collaborate with healthcare professionals and service providers to deliver high-quality care.
  • Manage employee schedules and facilitate communication among team members.

Requirements:

  • Current First Aid and CPR certification.
  • NDIS Worker Screening Check (Yellow Card) and Blue Card (Working with Children Check).
  • Queensland driver's license.
  • Tertiary qualifications and/or relevant experience in a health-related field.
