Job Title: Care Coordinator

 

We are seeking a skilled and experienced Care Coordinator to join our clinical team. This is an exciting opportunity for a motivated professional to make a real difference in the lives of our residents.

 

Key Responsibilities:

  • Providing expert clinical guidance and decision-making to promote best possible nursing practice.
  • Leading, coaching, mentoring, and supervising registered nurses to ensure high-quality care delivery.
  • Delivering personalized care services to residents, actively participating in quality management systems.
  • Promoting and coordinating clinical documentation to ensure accurate and up-to-date records.
  • Managing incidents promptly and effectively, minimizing risk to residents and staff.
  • Supporting residents, families, and teams in case conferencing, ensuring comprehensive care planning.

 

Requirements:

  • Relevant experience in senior nursing positions, with a proven track record of leadership and mentorship.
  • In-depth understanding of AN-ACC and quality assurance requirements, with the ability to apply this knowledge in a practical setting.
  • Current registration with AHPRA, with the right to live and work in Australia.
