Home-Based Therapist

This job involves providing essential home-based services for older clientele (65+) with a variety of health conditions, accessing services through their Home Care Package (HCP).
Key Responsibilities:- Deliver focused home-based services to enhance the independence and autonomy of clients.
- Work autonomously, managing your own caseload, primarily in Mildura, with occasional outreach to Swan Hill.
- Provide clinical supervision and support from the Clinical Lead of Occupational Therapy.
- Recognised tertiary qualification in Occupational Therapy.
- Current registration with AHPRA.
- Experience working in the Community Aged Care sector with clients receiving services through their Home Care Package.
- Demonstrated knowledge of clinical practice working with a variety of health conditions in 65+ year old clientele.

- Regular one-to-one clinical supervision and support from the Clinical Lead of Occupational Therapy.
- A current Working With Children Check card.
- Current and ongoing flu vaccinations.
- Australian working rights.
- Ability to pass a national police clearance and NDIS Clearance.
