About the Role

This is an exceptional opportunity to work in a supportive environment where you can make a real difference in the lives of our residents. As a Care Coordinator, you will be responsible for providing quality personal and social care to our elderly residents.

Key Responsibilities:
  • Providing Personal Care: Assisting with daily activities such as bathing, dressing, toileting, and grooming while maintaining the dignity and privacy of each resident.
  • Mobility Support: Helping residents with walking, using mobility aids, or transferring between different positions.
  • Monitoring Health: Observing and reporting any changes in the resident's health or behavior.
  • Supporting Mealtime: Ensuring residents receive their dietary requirements and assisting them during meals.
  • Maintaining Accurate Records: Keeping up-to-date records of care provided, observations, and changes in the resident's condition.
  • Collaboration: Working closely with healthcare professionals, including nurses, doctors, and other staff members, to deliver the best possible care.
Requirements:
  • A minimum of Cert III Individual Support (Ageing) or equivalent qualification.
  • An adaptable and passionate attitude, with a willingness to learn and grow with our organisation.
  • Excellent communication and teamwork skills, with the ability to build strong relationships with colleagues and residents.
