Job Title: Home Care Specialist
About the Role:This is a hands-on, client-facing position where you will work alongside a multidisciplinary team to deliver care within clients' homes. The successful candidate will be responsible for monitoring and assessing medication assistance, providing wound care support, continence management, and falls assessments.
Key Responsibilities:
  • Care Delivery: Deliver care within clients' homes in a hands-on, client-facing role.
  • Medication Assistance: Monitor and assess medication assistance.
  • Wound Care Support: Provide wound care support, continence management, and falls assessments.
  • Clinical Coordination: Collaborate with the team on all aspects of clinical care, coordination, and case management.

Requirements:
  • Qualifications: Bachelor of Nursing and current Nursing Registration with AHPRA.
  • Experience: RN experience within aged care in home/community service.
  • Transportation: Current Driver's Licence and own motor vehicle (car) with comprehensive insurance.
  • Vaccination: Covid-19 vaccination as per Department of Health requirements.
