About the Role

We are seeking a skilled and compassionate Care Coordinator to join our dedicated clinical team at Doutta Galla Queens Park, an aged care home located in Moonee Ponds.

Key Responsibilities
  • To deliver high-quality personalised care to residents, ensuring their health, well-being, and choice are maximised.
  • To work collaboratively with the clinical team to provide effective care coordination and support.
  • To maintain accurate records and communicate effectively with residents, families, and other healthcare professionals.

Requirements:

  • A current Division 1 AHPRA Nursing Registration.
  • Previous experience in Aged Care, preferably in a leadership or coordinating role.
  • Demonstrated ability to work effectively in a fast-paced environment and prioritise tasks efficiently.
  • Excellent time management skills, interpersonal, and communication skills.
