
Outpatient Care Specialist

Our organization is seeking an experienced Care Manager to join our Out of Hospital Care team in a permanent, full-time role.
- We require the successful candidate to work closely with external service providers and the OHC team to connect clients to appropriate supports and services that enable them to safely transition home after hospital.
The selected individual will be responsible for reviewing referral and intake information to complete assessments of OHC clients face-to-face or via phone within agreed time frames and performance indicators.
- They will undertake person-centred-care planning to identify care needs, develop care plans, explain rights and responsibilities, and collaborate with clients to set expectations.
- The candidate must identify key referral networks and access local resources consistent with OHC client care plans.
To be considered for this position, applicants must possess a diploma or degree in Health or Allied Health field or minimum of 6 years' relevant experience to equate to a health-related discipline.
- A minimum of two years' experience in a case management or client management role is also required.
- The selected candidate must provide evidence of full Covid-19 vaccinations.
- Strong communication, advocacy and interpersonal skills are essential for success in this role.
- The ideal candidate will demonstrate time management, prioritisation and budget management skills.
- Demonstrated experience in the provision of case management is highly desirable.
- A current Australian Driver's Licence and access to a reliable vehicle are mandatory.
- Familiarity with the Microsoft Office suite and ability to acquire skills for in-house client database systems is also expected.
