
Diabetes Care Specialist

As a leading provider of healthcare services, we are seeking an experienced Diabetes Care Specialist to join our team.
The Diabetes Using Our Strengths Service (DUOSS) is a pioneering virtual and face-to-face care model that focuses on the management of type 2 diabetes in Aboriginal and Torres Strait Islander Elders. The role will involve leading the clinical delivery of this innovative service, working in collaboration with a multidisciplinary team, including a Diabetes Educator and Aboriginal Peer Learning Officer.
To succeed in this role, you will require strong clinical skills in chronic disease management, as well as the ability to communicate effectively with Aboriginal and Torres Strait Islander people and facilitate culturally appropriate group discussions. You should also have experience working in a digitally enabled model of care and be comfortable using remote patient monitoring technologies.
The ideal candidate will possess the following qualifications and expertise:
- Demonstrated ability to work sensitively and effectively with Aboriginal and/or Torres Strait Islander people
- Registered Aboriginal Health Practitioner with unrestricted registration
- Clinical skills in chronic disease management and delivering care within a multi-disciplinary framework
Beyond your technical skills, you will bring a deep understanding of the cultural and social determinants that impact health outcomes for Aboriginal and Torres Strait Islander communities. This is a full-time fixed-term role, with some travel required to Bundjalung Country.
