Cancer Care Navigator Registered Nurse.
About the RoleThis is an opportunity to lead and support our front-line team members, building your clinical leadership skillset in oncology. You will be responsible for facilitating the continuity and quality of care of patients diagnosed with cancer and their families, acting as a key hospital resource for promoting best practice in patient care through clinical improvement initiatives and service innovation.
Responsibilities- Providing navigation across the patient's cancer care pathway to assess and monitor a comprehensive care plan which includes assessment, planning, treatment, supportive care, survivorship and/or palliation in collaboration with team members.
- Collaborate with other teams and act as a conduit between patients and family, specialist/s, hospital and community providers.
- Promote an end-to-end pathway across services in and out of the hospital setting.
Your Key Skills and Experience:
- Current registration with AHPRA as a Registered Nurse.
- Minimum of three years post-registration experience in Oncology nursing or relevant advanced practice experience.
- Understanding of scope of practice in line with relevant registration.
- Demonstrated experience in continuous improvement and initiatives supporting the customer experience.
- Demonstrated ability to work effectively in a multidisciplinary team environment and independently as required.
- Highly effective communication and interpersonal skills.
- Proven project management skills with sound organisational and change management skills.
- Intermediate to advanced computer skills.
