About the Role
  • Respond to referrals, conduct initial needs assessments and develop individualized case plans to deliver trauma-informed, early intervention case work.
  • Provide practical support, facilitate referrals to internal and external services, and coordinate group programs to promote positive outcomes.
  • Maintain accurate and timely case documentation, including risk assessments, progress reports, and data entry to ensure effective service delivery.
Key Responsibilities
  • Work collaboratively with internal teams and external agencies to advocate for clients and participate in case reviews.
  • Sustain relationships with clients and stakeholders to provide ongoing support and guidance.
Requirements for Success
  • Relevant qualification and/or experience in the human services industry.
  • Knowledge and experience working with client issues, including domestic violence, substance abuse, mental health.
  • Excellent oral and written communication skills, with the ability to maintain accurate case documentation and engage effectively with clients and external agencies.
  • Current NSW driver's license and current First Aid and CPR Certificate.
