Job Title

Seeking a dedicated and experienced Care Coordinator to join our team

As a key member of our organization, the successful candidate will be responsible for coordinating and managing the delivery of care services to clients. This is an exciting opportunity for a motivated and enthusiastic individual who is passionate about delivering high-quality care.

The ideal candidate will have experience in home care, with a strong understanding of care planning and client assessments. They will also possess excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and other stakeholders.

Key Responsibilities:

  • Conducting thorough client care assessments and developing personalized care plans
  • Managing client compliance and case loads
  • Responding to new client inquiries and scheduling care consultations
  • Performing tasks in accordance with established routines, methods, standards, and procedures
  • Assisting clients in achieving planned outcomes and providing support with daily living activities
  • Coordinating the provision of quality person-centred care according to client needs and service/care plans
  • General administration duties including recruitment of staff and business development

Requirements:

  • Experience in home care: 3 years (Required)
  • Australian Driving License and Comprehensive car insurance
  • Cert IV in Aged Care or Disability Support or Health Care

Benefits:

  • Competitive hourly rate: $37.00 - $40.00
  • Performance-based bonuses

Work Environment:

  • Permanent and casual positions available
  • Monday to Friday, day shift
