Complex Case Manager Job Opportunity

We are seeking experienced and skilled professionals to fill a Complex Case Manager position. As a key member of our team, you will be responsible for providing high-quality support and guidance to clients with complex needs.

The Role

  • Manage a caseload of complex clients and provide expert advice and support as required.
  • Utilize your experience in case management/clinical settings to work effectively with clients presenting with mental health conditions, vulnerabilities, and/or complex co-morbidities.
  • Apply the social determinants of health and wellbeing to inform your practice and ensure that all aspects of client care are considered.

Key Responsibilities

  • Conduct assessments of client referrals made where mental health issues are evident.
  • Record accurate and comprehensive case notes and other records in a case management system.
  • Negotiate client claim outcomes with relevant stakeholders.
  • Perform urgent risk assessments with clients when necessary.
  • Provide expert guidance and support to non-clinical staff undertaking assessment of client referrals.
  • Establish effective communication channels with clients through phone calls or other means.
  • Link clients to appropriate mental health services.

Requirements

  • Tertiary qualification in a relevant health field.
  • Registration with AHPRA or AASW is highly desirable.
  • At least 2 years' experience working as a case manager or equivalent clinical role.
  • Substantial experience working within a mental health or community case management setting.
