No more shift work, all day shifts between Monday - Friday. This is a Permanent position with guaranteed hours and stability in our Home Care team.
Manage the Clinical Care of clients in the Lower Central Coast Region for HammondCare at Home. This is a Full Time position across Monday to Friday 9.00 am -5.00 pm.
Responsible for the clinical assessment of each Home Care Package client, ongoing clinical support/advice and hands-on clinical care as required. Support with COVID escalations and care worker education.
The Clinical Care Manager leads, supports, and mentors their team.
This role requires professional accountability and responsibility in line with the registration requirements of a Registered Nurse as the role is responsible for the management and direct care of clients' clinical needs.
As a RN you will be working with a collaborative team environment that values patient centred care, ensuring positive health outcomes for all patients.
Our ideal candidate has excellent communication skills, ability to work autonomously and as part of a team, providing high-quality patient care services to meet the needs of our clients.
Key performance indicators include but not limited to timely completion of tasks, accurate documentation and maintaining confidentiality in all interactions.
