Job Description

This role focuses on supporting participants with complex needs to achieve their NDIS goals by overcoming barriers, managing transitions, and connecting with appropriate resources and providers. You will oversee and actively manage care plans, ensuring outcomes are met while promoting participant independence and choice.

The ideal candidate will have a strong understanding of and commitment to Aboriginal Community Control and self-determination, as well as knowledge of the unique challenges faced by Yolngu people in remote settings. Essential requirements include a relevant bachelor's degree (or higher) in disability, health, or social work; experience in Support Coordination or case management within the NDIS, Disability, or Health sector; an NT Driver's License; and the ability to travel to remote communities.
