Key Responsibilities:
  • Conduct comprehensive nutritional assessments for residents and clients in their own homes.
  • Develop personalized nutrition plans and goals to support clients' dietary needs.
  • Work holistically with other allied health professionals to provide a multidisciplinary care plan.
  • Liaise with service providers to ensure effective communication and coordination of care.
  • Manage food intolerances and allergies, and provide education on diabetes management.
  • Educate clients and their families to optimize their ability to live healthy lives within the community.
Requirements:
  • Bachelor of Nutrition and Dietetics (or equivalent) degree.
  • Experience working within home care, aged care, and NDIS settings.
  • Membership with the Dietitians Association of Australia with Accredited Practicing Dietitian status.
  • Police Check Clearance.
  • Excellent verbal and written communication skills.
  • Strong time-management skills and attention to detail.
  • Ability to work independently and as part of a team.
