NDIS Support Coordination Role

We are seeking an experienced NDIS Support Coordinator to fill a full-time ongoing position.

This role will support participants and their families in understanding their NDIS plan, goals, and objectives while promoting choice and control and supported decision making.

You will coordinate mainstream, informal, and formal supports to assist participants in achieving their goals. Key skills required include providing Level 2 Coordination of Supports, completing reports, monitoring progress against NDIS goal plans and budgets, and building connections with providers and local community services.

Key Requirements:

  • Current unrestricted VIC Driver's license.
  • Australian citizenship or permanent residency.
  • Maintaining a current First Aid certificate.
  • Working with children permit.
  • Satisfactory national police check.
  • Pre-employment medical and fitness report.
  • NDIS Worker Screening and NDIS Orientation Module.
