We are seeking an experienced and skilled Practice Nurse Team Leader to join our team at For Health Medical Centres.
Key Responsibilities:- Direct Patient Care Activities: Conduct a range of direct patient care activities in the treatment room and in accordance with company Standard Operating Procedures (SOPs) and/or Standing Orders.
- Chronic Disease Management: Assist the GP with conducting health assessments, GP management plans, team care arrangements, reviews, and initiate other chronic disease management tasks in accordance with Medicare requirements.
- Nurse Led Clinics: Collect health history, plan, coordinate, and document patient care, and evaluate patient outcomes related to patient condition(s) and/or presentations in accordance with Primary Health Care Limited (PHC)-endorsed policies and standing orders.
- Nursing Leadership: Attendance at meetings both internally and externally, where Medical Centre nursing representation is required. Delegation of operational tasks to ensure efficient nursing service at centre.
- Current AHPRA registration as a Registered Nurse (RN).
- At least 2 years' experience providing direct clinical nursing care.
- Experience in a primary care setting and understanding of chronic disease management, prevention principles, and health promotion, desirable.
