Job Overview

We are seeking a dedicated professional to play a key role in enhancing the lives of older Australians.

The successful candidate will conduct thorough assessments to identify the needs of older adults, and facilitate referrals to services that promote independent living.

Main Responsibilities
  • Conduct comprehensive assessments to determine the needs of older adults.
  • Develop and implement personalized care plans to support clients in maintaining their independence.
  • Collaborate with families and carers to ensure they are engaged in the assessment process and receive ongoing support.
  • Ensure all assessments are conducted in line with relevant industry standards and regulations.
Key Qualifications
  • Relevant tertiary education in a health-related discipline, such as nursing, occupational therapy, physiotherapy, or social work.
  • At least 1 year of experience in aged care, including client assessments and/or service delivery.
  • Current unrestricted General Registration with AHPRA or relevant professional association.
  • Proficiency in Microsoft Office, including Word, Excel, and Outlook.
