We are seeking a skilled and experienced Aboriginal Health Practitioner to deliver high-quality, culturally safe cardiac healthcare services that cater to the unique needs of Aboriginal and Torres Strait Islander communities.
About the Position- The successful candidate will be responsible for providing comprehensive assessments, developing individualized care plans, and delivering evidence-based treatments that promote positive health outcomes.
- This role requires collaboration with multidisciplinary teams to ensure seamless care coordination and effective program implementation.
- To conduct thorough assessments and develop tailored care strategies that prioritize patient-centered care.
- To design and implement holistic health programs that address the specific needs of Aboriginal and Torres Strait Islander communities.
- To cultivate strong relationships with community members and healthcare providers to ensure smooth communication and care coordination.
To be successful in this role, you will need:
- A relevant tertiary qualification (Bachelor or Master level) in a field such as nursing, health science, or a related discipline.
- Demonstrated experience working with Aboriginal and Torres Strait Islander communities, with a strong understanding of their cultural protocols and practices.
- Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
