This new role will support the delivery of chronic disease management (CDM), helping patients navigate long-term health concerns through high-quality care planning, health assessments, and follow-up support.
You'll work closely with our GPs, allied health staff, and the broader care team to coordinate and deliver compassionate, person-centred care, supporting prevention, education, and patient empowerment.
