Care Coordinator

We are seeking a dedicated and skilled Registered Nurse to join our Transit Lounge team at Tweed Valley Hospital.
This role involves delivering high-quality patient care, working collaboratively with multidisciplinary teams, and providing education and support to patients, families, and carers.
Key Responsibilities:- Coordinate person-centered nursing care in accordance with Nursing and Midwifery Board of Australia Standards for Practice for Registered Nurses.
- Work effectively with healthcare professionals to achieve positive patient outcomes.
- Provide health education and support to patients, families, and carers on health-related issues.
- Maintain accurate and timely documentation of patient information.
- Contribute to quality improvement activities and maintain a safe and supportive work environment.
- Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency.
- Relevant experience in a hospital or healthcare setting, preferably in a transit lounge or emergency department.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and staff.
- Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
- Sound knowledge of infection control principles and procedures.
