The organization is seeking a highly skilled and compassionate Registered Nurse to provide high-quality clinical care and support services to individuals in their homes.

About the Role:
  • Ensure alignment with Aged Care Quality Standards and organizational policies.
  • Develop individualized care plans, working closely with participants, carers, and stakeholders to achieve goals and improve outcomes.
  • Work collaboratively as part of a multidisciplinary team for comprehensive assessments, care coordination, and education.
  • Utilize strong case management skills, effective communication, and problem-solving abilities to manage complex cases.
Requirements:
  • Current registration with the relevant health authority.
  • Valid drivers' license and NDIS Worker Screening Clearance.
  • CPR Certificate (annual certification required).
  • National Police Check.
  • Strong knowledge and experience working with older adults, particularly those with dementia and their carers.
  • Excellent written and computer skills for document creation and reporting.
