We're seeking a dedicated and compassionate Care Coordinator to join our team.
About the RoleThis full-time role involves working 38 hours per week, Monday to Friday. You will support clients in the Goulburn region and play a vital role in providing high-quality care to those living at home.
The ideal candidate will have excellent communication skills and be able to work independently, with a strong focus on delivering clinical care and conducting comprehensive assessments and regular reviews.
Key Responsibilities- Delivering Clinical Care: Provide high-quality nursing care to clients in their own homes.
- Conducting Comprehensive Assessments: Gather information about clients' medical history, current health status, and social circumstances.
- Care Planning and Coordination: Work with clients, families, and healthcare professionals to develop and implement individualized care plans.
- Advocating for Clients: Ensure clients receive fair and respectful treatment, and advocate on their behalf when necessary.
- Providing Health Education: Educate clients and their families about healthy living, disease prevention, and management of chronic conditions.
