Key Responsibilities- Conduct initial and ongoing assessments of clients' circumstances, making recommendations to resolve complex and unique situations.
- Provide direct client contact and clinical care, as well as advocacy work via telehealth or face-to-face interactions.
- Manage a portfolio of mixed clients with diverse needs, including dementia, housing, access to services, guardianship, or elder abuse.
- Collaborate with a multidisciplinary team to provide holistic care to clients and liaise with their carers, case managers, and other health partners.
