Program Support Specialist

We are seeking an experienced and motivated individual to join our team as a Program Support Specialist.

The ideal candidate will have previous experience working with the disability, health or aged care industry in a case management role. Understanding of the "young people in residential aged care" program is advantageous.

This is an exciting opportunity to provide support to individuals with significant and permanent disabilities and their families and carers.

Key Responsibilities:

  • Collaborate with hospital staff to ensure participants who are medically ready for discharge have the right supports and accommodation in place when returning to the community.
  • Work closely with planners, participants, and hospital staff to achieve optimal outcomes.
  • Manage confidential and sensitive information with discretion and professionalism.

Benefits:

This is a fixed-term contract position providing an hourly rate of $53.63 plus superannuation.
