Job Overview

We are seeking an exceptional Clinical Care Coordinator to lead our clinical and care teams in delivering exceptional care to residents while fostering their unique identities and experiences.

This role plays a pivotal part in our home's clinical governance, overseeing specialized portfolios such as education, palliative care, and infection control.

Key Responsibilities:
  • Comprehensive Care Provision: Ensure residents receive tailored nursing care, documented and delivered by our skilled staff.
  • Resident-Centric Decision-Making: Prioritize resident preferences, independence, dignity, and care-related risks when making decisions.
  • AN-ACC Reappraisals: Conduct scheduled reappraisals, including mandatory and voluntary assessments, as required.
  • Nurse Accountability: Hold Registered Nurses (RNs) and Enrolled Nurses (ENs) accountable for case reporting, care planning, and addressing patient concerns promptly.
  • Clinical Assessments: Perform routine clinical assessments to address identified gaps in care.
About You:
  • You must be currently registered as a Nurse with AHPRA, with no restrictions.
  • Demonstrate senior management experience in the aged care sector.
  • Show in-depth knowledge of the aged care industry, with a keen eye for emerging trends and best practices.
  • Demonstrate proficiency in managing human resource matters and possess strong negotiation skills.
  • Show proficiency in utilizing various information technology tools and software, including Word and Excel, with a quick ability to adapt to new skills as required.
