HammondCare invites a passionate and experienced professional to assume the role of Dementia Consultant. The successful candidate will play a vital part in delivering our Support Program, collaborating with colleagues to enhance lives and implement best practices.

Key Responsibilities:
  • Support families, caregivers, and clinical staff in understanding how dementia impacts each individual.
  • Equip carers and clinicians with skills to prevent and respond to behaviour changes through modelling, coaching, and resource development, including behaviour support planning.
  • Develop contextualised reports that drive change on the ground and improve quality of life for people living with dementia.
  • Conduct comprehensive assessments that understand the holistic experience of individuals and their care networks.
  • Work collaboratively with the broader HammondCare team to build capability across home and aged care sectors.

What We Require from You?

  • Expertise in supporting individuals living with dementia.
  • Experience in Australian residential and community aged care settings.
  • Ability to share knowledge in a relatable manner, facilitating change at all levels.
  • High level of negotiation, communication, and relationship-building skills.
  • Excellent problem-solving and decision-making abilities.
  • Capacity to prioritise multiple tasks and meet deadlines.
  • Strong written and verbal communication skills.
