Job Title:

The Outreach Practitioner plays a pivotal role within our multidisciplinary team, contributing to the delivery and development of health services for Marginalised and Vulnerable Adults in Suffolk.

Key Responsibilities include:

  • Managing client cases, assessing needs and risk, and delivering outreach services to hard-to-reach groups.
  • Care will be delivered within the framework of NICE guidelines and evidence-based best practice.
  • Liaising and working with partners serving clients from various groups, including asylum seekers, refugees, and migrant workers.
  • Contribution to multi-disciplinary comprehensive assessments to establish eligibility for health and social care services.
  • Undertaking continuous risk assessment and contribution to effective risk management.
Requirements:
  • Registered Nurse or equivalent qualification and experience.
  • Knowledge of Mental Health legislation and ability to apply it.
  • Experience of working with diverse client groups, particularly those who are marginalised and vulnerable.
  • Ability to work autonomously and as part of a team.
  • Excellent communication and interpersonal skills.
